Sex recorded at birth is optional. Pregnancy and pregnancy planning are asked separately when relevant.

Selected conditions can bring additional vaccine topics into view. You can skip anything that does not apply.

Mark vaccines you recall, then review each topic and its source with a clinician or public health unit.

Where guidance comes from

Ontario publishes the routine schedule and provincial program information. NACI provides national clinical recommendations, reflected in the Canadian Immunization Guide. A recommendation does not always mean a vaccine is publicly funded for every person in Ontario.

Privacy and limits

Your answers are used in this browser session to show the guide. This site does not ask you to create an account or save a health profile. If you download a summary, it contains the answers you chose to share, including the optional sex detail. Keep that file somewhere private.

The guide does not cover every condition, travel or workplace risk. Previous doses, products, dates and eligibility matter. Bring your vaccine record to a health professional for an individual plan.
